Athletic Clearance
 
The following is a list of sports teams St. Lucy's fields during the school year and when the paperwork is due for that sport or season:
FALL:
Volleyball, Tennis and Cross Country
Clearance papers for due by August 6, 2010
WINTER:
Basketball, Soccer and Water Polo
Clearance papers due by October 1, 2010
SPRING:
Track, Swimming and Softball
Clearance papers due by January 14, 2011

Any athlete who is not planning to participate in any St. Lucy’s summer sports program, must have paperwork in by the above dates. Athletes who plan to participate in St. Lucy’s summer programs, all paperwork must be turned in to the gym office by June 7, 2010. (REMINDER: Your paperwork is good for one school year. Those who turn in all paperwork will be cleared for the entire 2010-2011 school year.)

Athletic Fee: During the school year, each athlete is assessed a $75 Athletic Fee and is asked to participate in a fundraiser put on by each sport. This helps to defray the cost such expenses as officials, facilities rentals, sport specific equipment and buses. In addition, the purchasing of uniforms and individual equipment will be the student’s responsibility.

Mandatory Parent Meeting: Prior to each season a meeting will be held to discuss a variety of items pertinent to that season. In order to participate in a sport, the athlete and least one parent must attend. These dates are as follows:

  • Cross Country, Tennis and Volleyball – Wednesday, September 1, 2010 - 6 PM
  • Basketball, Soccer and Water Polo – Wednesday, November 10, 2010 - 6 PM
  • Softball, Swimming and Track – Wednesday, January 12, 2011 - 6 PM
ATHLETICS CLEARANCE PROCEDURE

The following forms must be downloaded, completed and returned by the appropriate date in order for an athlete to try out for a sport. All forms must be in the original form – NO FAXES OR COPIES ARE ACCEPTED. No student will be allowed to try out if all forms are not in by the deadline!

The following St. Lucy’s Sports Clearance Forms must be COMPLETELY filled out and returned to the athletic office:

  Sports physical form signed and stamped by the doctor. (Summer time is a good time to take care the physical and paperwork. Sports physicals are good for one year. If the athlete takes care of it during the summer, it will be valid for the entire school year!)
